(a) An apprentice direct-entry midwife permit is valid for two years from the date of issue.
(b) An individual applying for renewal of an apprentice direct-entry midwife permit shall
(1) apply on a form provided by the
department;
(2) pay the fees
established in
12 AAC 02.145; and
(3) document continued qualification under
12 AAC 14.130.
