Chapter 660-5-21 - PUTATIVE FATHER REGISTRY
Section 660-5-21-.02 - Putative Father Registration

(1) A person filing an acknowledgment of paternity or filing a notice of intent to claim paternity with the Putative Father Registry must supply the information listed below on the putative father intent to claim paternity registration form:

(a) Putative father's name, social security number, date of birth and current address.

(b) The mother's name, including other names that have been used, social security number, date of birth, and address, if known.

(c) The child's name and place of birth, if known.

(d) The possible dates of sexual intercourse.

(2) The putative father shall also submit an affidavit of current income and other financial information with the putative father intent to claim paternity registration form. The affidavit shall include the following information on the putative father:

(a) his social security number.

(b) his current employer if any and the employer's name and address.

(c) if unemployed, a statement that he is currently unemployed.

(d) title of last job position.

(e) average monthly salary last year.

(f) current gross monthly salary divided into employment income, self-employment income, other employment-related income and other non-employment related income.

(g) monthly child care expenses.

(h) whether health care insurance is being provided to the child(ren) and the amount paid monthly for the insurance coverage.

(3) When incomplete information is received, the registrant will be notified in writing by certified mail, return receipt requested. The registrant will have 15 days from the date of the written notice to provide complete information. Should this not be received, the name will not be entered on the Registry.

(4) A person who has previously registered, shall subsequently notify the Putative Father Registry of a change of address on a notarized form supplied by the Department.
