Section 540-X-3-.17 - Renewal Of Certificate Of Qualification Without Examination (Limited License)

Physicians must apply to the Board to renew their limited certificate of qualification by January 1 of each year. The Board shall have full discretion to accept or reject the application for renewal. Physicians must pay a renewal fee of $15.00, or as is otherwise set by law.

Rule 540-X-3-.16 was previously numbered Rule 540-X-3-.15 as of the certification filed August 21, 1996. Previous rule 16 was renumber to .17 as per certification filed October 15, 2008; effective November 19, 2008.
