Section 485-X-4-.09 - Lost Or Destroyed Seals Or Authorization For Registered Interior Designer

Lost or destroyed seals or Authorization shall be replaced by written request to the Board on an application as approved by the Board. A fee in an amount to be determined by the Board shall be due together with the application. Such replaced seal or Authorization shall bear the same registration number, the full name of the applicant, the date and term of issuance, the seal of the Board, the signature of the Chairman, and the same date and number as the original and shall be marked "Duplicate."
