Chapter 445-X-1 - HEARING INSTRUMENT DEALERS
Section 445-X-1-.10 - Receipts

(1) Any individual fitted with or purchasing a hearing instrument shall be furnished with the following:
(a) A bill of sale or receipt which
shall contain:
1. The licensee's name, license
number or permit number, business name, and street address.
2. The make, and model, of the hearing
instrument.
3. Guarantee and
warranty terms.
4. Full terms and
conditions of the sale; including the return policy, if any.
5. If the hearing instrument is not new, the
receipt shall be clearly marked as "used", "reconditioned", or "rebuilt". No
other terms shall be used to describe a hearing instrument that is not new. The
receipt shall also include warranty/guarantee terms or a statement that the
item is not under warranty/guarantee.
(b) A "User's Instructional Brochure" as
supplied by the manufacturer.
