Section 380-X-3-.09 - Replacing Lost, Destroyed Or Mutilated Licenses

The Board shall issue a new License identified by the same number as that of the original to replace any license lost, destroyed, or mutilated upon receipt by the Board of a written request and a fee of fifty dollars from the licensee, provided the Board's records show the original license is valid. This new license shall bear the signatures of current officers, the date of reissue, and below the license number the following: "Original license approved _____________ (date)."
