An emergency clearance is being requested for information needed to assess the effectiveness of pain care management provided to veterans.

The Department of Veterans Affairs (VA) requests that eligible entities interested in applying for funding under the Supportive Services for Veteran Families (SSVF) Program submit a letter of intent. The SSVF Program expects to publish a notice of funding availability (NOFA) in fiscal year (FY) 2013. VA has undertaken an initiative to implement a patient-centered medical home model, ``Patient Aligned Care Team'' or PACT at all Veterans Health Administration (VHA) Ambulatory Primary Care sites. This initiative supports the VHA's Universal Health Care Services Plan to redesign VHA healthcare delivery through increasing access, coordination, communication, and continuity of care. The patient experiences from this data collection are intended to help form future national VA policy.

Pursuant to the National Environmental Policy Act (NEPA) of 1969, as amended, (42 U.S.C. 4331 et seq.), the Council on Environmental Quality Regulations for Implementing the Procedural Requirements of NEPA (40 CFR parts 1500-1508), VA's Implementing Regulations (38 CFR part 26), as well as the settlement agreement resulting from Planning Association for Richmond, et al. v. U.S. Department of Veterans Affairs, C-06-02321-SBA (filed 6 June 2008), VA has prepared a Draft EIS for the proposed implementation of the SFVAMC LRDP in San Francisco, California. The SFVAMC LRDP involves development and construction of patient care buildings, research buildings, business occupancy buildings, and parking structures, as well as retrofitting seismically deficient buildings. The Draft EIS identifies and evaluates environmental factors associated with new construction, demolition, as well as seismic retrofit to upgrade the SFVAMC for purposes of meeting the needs of Veterans of the North Coast and San Francisco Bay Area over the next 20 years.
