Title: Community Rating System (CRS) ProgramApplication Worksheets and Commentary. OMB Number: 1660-0022. Abstract: CRS Application Cover Page, Recertification Worksheet and Modification/Cycle Cover Page are used by a community to apply or renew CRS program participation and specify what activities will be performed by the community. CRS Activity Worksheets are used by the community to provide details in the selected activities based on the CRS Coordinator's Manual. Affected Public: State, Local or Tribal Government. Number of Respondents: 150 for application and 950 for maintenance. Estimated Time per Respondent: 31 hours for application and 4 hours for maintenance. Estimated Total Annual Time Burden: 8,450 hours. Annual Frequency of Response: 1. Title: Federal Emergency Management Agency Housing Inspection Services Customer Satisfaction Survey. OMB Number: 1660-NW31. Abstract: FEMA Housing Inspection Services contracts inspectors to assess dwelling damage and verify personal information of applicants for FEMA disaster assistance in federally declared disasters areas. Because FEMA needs to evaluate the inspectors' performance, FEMA conducts surveys to measure the satisfaction level of the applicants with their inspection experience. FEMA Inspection Services Managers and Task Monitors generally use the survey results to gauge and make improvements to disaster services that increase customer satisfaction and program effectiveness. The information is shared with Regional staff specific to the federal declaration for which the survey is conducted. Affected Public: Individuals and households. Number of Respondents: 10,608. Estimated Time per Respondent: 0.25 hours. Estimated Total Annual Time Burden: 2,652 hours. Annual Frequency of Response: 1.
