The form number, if applicable: Not applicable. 5. How often the collection is required or requested: Annually for most reports and at license termination for reports dealing with decommissioning. 6. Who will be required or asked to respond: NRC licensees and Agreement State licensees, including those requesting license terminations. Types of licensees include civilian commercial, industrial, academic, and medical users of nuclear materials. Licenses are issued for, among other things, the possession, use, processing, handling, and importing and exporting of nuclear materials, and for the operation of nuclear reactors. 7. The estimated number of annual responses: 53,483 (6,219 NRC licensees + 47,264 Agreement State licensees). 8.
