Abstract: VA Form 21P–8941 is primarily used to gather the necessary information to determine a claimant’s continued eligibility for REPS benefits. The information on the form is necessary when a claimant has an income that is at, or near, the allowable limit for income. The form is returned by mail or in person to certify REPS eligibility requirements. Once the form is received, claim processors review the information provided and assess whether the claimant is eligible for REP benefits. Without this information, determination of continued entitlement would not be possible. This is an extension with no substantive changes to the form. There has been no burden change since the last approval. An agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a currently valid OMB control number.
