Abstract: VA Form 21–10210 is used
by the claimant to gather lay or witness
statements that support an existing
claim for benefits/services. Without this
information, VA may not be able to
efficiently and successfully process
claims that may require additional
statements associated with a claim for
benefits/services. No changes have been
made to this form. The respondent
burden has decreased due to the
estimated number of receivables
averaged over the past year.

Type of Review: Extension of a
currently approved collection.
Abstract: VA Form 21P–8941 is
primarily used to gather the necessary
information to determine a claimant’s
continued eligibility for REPS benefits.
The information on the form is
necessary when a claimant has an
income that is at, or near, the allowable
limit for income. The form is returned
by mail or in person to certify REPS
eligibility requirements. Once the form
is received, claim processors review the
information provided and assess
whether the claimant is eligible for REP
benefits. Without this information,
determination of continued entitlement
would not be possible. This is an
extension with no substantive changes
to the form. There has been no burden
change since the last approval.
An agency may not conduct or
sponsor, and a person is not required to
respond to a collection of information
unless it displays a currently valid OMB
control number. The Federal Register
Notice with a 60-day comment period
soliciting comments on this collection

processed by VA. Statements submitted

contain certification by the respondent

and correct. This form facilitates claims
processing by providing a uniform
format for the certification statement.
No changes have been made to this
form. The respondent burden has
increased due to the estimated number
of receivables averaged over the past
year.
