The information collected via the DD– 3112 will inform decisions made about the status of DoD facilities and spaces that Affected Individuals have entered. This information may be used to make decisions to protect the health and safety of DoD personnel and facilities. The DD Form 2769, ‘‘Application for Annuity-Certain Military Surviving Spouses,’’ used in this information collection, provides a vehicle for the surviving spouse to apply for the annuity benefit.
