sign and date to acknowledge his or her
understanding of the notification.

respondents and the amount of time
estimated for an average respondent to
respond: An estimated 300 respondents
will provide information to complete
this form once annually, and it will take
approximately 15 minutes to complete
the form.

or not hiring a candidate for the
criminal investigator (special agent)

will utilize the form annually, and it
will take each respondent
approximately 45 minutes to complete
their responses.

285 hours, which is equal to 380 (total
respondents) * 1 (# of response per
