Abstract of Proposed Collection
The Application for a U.S. Passport:
Corrections, Name Change Within 1
Year of Passport Issuance, And Limited
Passport Holders (DS–5504) is the form
used by current passport holders who
need to re-apply for a passport, at no
charge.
This form has been amended based on
a change in Department policy. The
Department’s new policy permits
passport applicants to select the gender
marker on their passport without
presenting medical documentation of

gender transition. This policy change
includes updating forms to add a third
gender marker ‘‘X’’ for applicants
identifying as non-binary, intersex, and/
or gender non-conforming (in addition
to the existing ‘‘M’’ and ‘‘F’’ gender
markers).

Methodology
Passport Services collects information
from U.S. citizens and non-citizen
nationals when they complete and
submit the DS–5504, ‘‘Application for a
U.S. Passport: Corrections, Name
Change Within 1 Year of Passport
Issuance, And Limited Passport
Holders’’. Passport applicants can either
download the DS–5504 from the
internet or obtain the form from an
Acceptance Facility/Passport Agency.
The form must be completed, signed,
and be submitted by mail (or in person
at Passport Agencies domestically or
embassies/consulates overseas).

To Determine That an Applicant’s
Entry is in the National Interest
By virtue of the authority vested in
the Secretary of State by the laws of the
United States, including Section 1 of the
State Department Basic Authorities Act,
as amended (22 U.S.C. 2651a) and the
Presidential Proclamation of May 29,
2020 titled Suspension of Entry as
Nonimmigrants of Certain Students and
Researchers From the People’s Republic
of China, I hereby delegate to the
Assistant Secretary for Consular Affairs,
to the extent authorized by law, the
authority under section 2(a)(vii) of said
Presidential Proclamation to determine
that an applicant’s entry into the United
States would be in the national interest.
The Secretary, Deputy Secretary,
Deputy Secretary for Management and
Resources, and the Under Secretary for
Management may exercise any function
or authority delegated by this
delegation. The authorities delegated
herein may be re-delegated, to the extent
authorized by law.
This Delegation of Authority does not
supersede or otherwise affect any other
delegation of authority currently in

Notice of request for public
comment.
ACTION:
The Department of State is
seeking Office of Management and
Budget (OMB) approval for the
information collection described below.
In accordance with the Paperwork
Reduction Act of 1995, we are
requesting comments on this collection
from all interested individuals and
organizations. The purpose of this
notice is to allow 60 days for public
comment preceding submission of the
collection to OMB.
DATES: The Department will accept
comments from the public up to
November 15, 2021.
