Financial Management Service (FMS)
have consolidated to become the Bureau
of the Fiscal Service (Fiscal Service).
Information collection requests
previously held separately by BPD and
FMS will now be identified by a 1530
prefix, designating Fiscal Service.
Form Number: FS Form 4000.
Abstract: The information is
requested to support a request to reissue
paper (definitive) Series EE, HH, and I
United States Savings Bonds;
Retirement Plan Bonds; and Individual
Retirement Plan Bonds and to indicate
the new registration required.
