In compliance with the
Paperwork Reduction Act of 1995, the
Department of the Air Force announces
a proposed public information
collection and seeks public comment on
the provisions thereof. Comments are
invited on: (a) Whether the proposed
collection of information is necessary
for the proper performance of the
functions of the agency, including
whether the information shall have
practical utility; (b) the accuracy of the
agency’s estimate of the burden of the
proposed information collection; (c)
ways to enhance the quality, utility, and
clarity of the information to be
collected; and (d) ways to minimize the
burden of the information collection on
respondents, including through the use
of automated collection techniques or
other forms of information technology.

Needs and Uses: The information
collection requirement is necessary to
enable the request of advance funds for
purchase of supplies for chapel projects,
or for the payment of contract payments
to Non-personnel Service Contracts
between the local base chapel and each
individual contractor. Air Force
Instruction 52–105V2 requires that all
contract payments only be
accomplished by EFT, the 4360 Form
gives CCAC the information needed to
pay by EFT.
Affected Public: Individuals or
Households.
Annual Burden Hours: 6,250 hours.
Number of Respondents: 5,000.
Responses per Respondent: 5.
Annual Responses: 25,000.
Average Burden per Response: 15
minutes.
Frequency: Annually.
The Chaplain Corps Accounting
Center (CCAC) requires the forms to be
completed and submitted, to have all
the information needed to process fund
requests and payments. The calculation
of average burden per response uses
