Back Services Form.
(3) Agency form number: 1–796.
(4) Affected public who will be asked
or required to respond, as well as a brief
abstract: Primary: This form is utilized
by authorized agencies to enroll
individuals in the Rap Back Service to
ensure the submitting agency is notified
when individuals in positions of trust
engage in criminal conduct or
individuals under the supervision of a
criminal justice agency commit
subsequent criminal acts.
(5) An estimate of the total number of
respondents and the amount of time
estimated for an average respondent to
respond: It is estimated that 120,000
respondents will complete each form
within approximately 5 minutes.
(6) An estimate of the total public
burden (in hours) associated with the
collection: There are an estimated 500
total annual burden hours associated
with this collection.
