PRA of 1995 (Pub. L. 104–13; 44 U.S.C.
3501–3521), Federal agencies must
obtain approval from the Office of
Management and Budget (OMB) for each

or sponsor. This request for comment is
being made pursuant to Section
3506(c)(2)(A) of the PRA.
With respect to the following
collection of information, VHA invites
comments on: (1) Whether the proposed
collection of information is necessary
for the proper performance of VHA’s
functions, including whether the
information will have practical utility;
(2) the accuracy of VHA’s estimate of
the burden of the proposed collection of
information; (3) ways to enhance the
quality, utility, and clarity of the
information to be collected; and (4)
ways to minimize the burden of the
collection of information on
respondents, including through the use
of automated collection techniques or
the use of other forms of information
technology.

Abstract: The death rate for Veterans
will continue to grow as the number of
Veterans 85 years of age and older is
projected to increase by 32 percent
between 2009 and 2018. Given this
trend, the VA will face substantial
challenges in providing care to Veterans
near the end of life. For example,
extensive data from non-VA health care
systems demonstrates that physical
symptoms like pain, dyspnea and
nausea are common in advanced illness,
but are under-recognized and
inadequately managed. Other studies
have found that providers often lack the
time and communication skills to
discuss goals of care and treatment
preferences with patients and families;
there is strong evidence that when
providers fail to discuss goals of care
with patients and families, patients
often receive unwanted, aggressive lifesustaining treatment that is not
consistent with their preferences. A
related problem has been the high
incidence of deaths in an acute care
setting. As many Veterans approaching
end of life may prefer a more quiet and
comfortable setting than can be
provided in acute care, with improved
communications and availability of
services much of this end of life care
could be shifted to a VA hospice unit or
to hospice in the Veteran’s home. The
VA has been and continues to be a
leader among healthcare systems in the
provision of hospice and palliative care.
