injuries and illnesses for medical
treatment and payment of claims. It
assists the Exchange in recouping
damages, correcting deficiencies,

Affected Public: Exchange employees,
customers, guests, visitors, and
members of the public.
Annual Burden Hours: 4,854.
Number of Respondents: 4,854.

Average Burden per Response: 1 hour.
