Title: REPS Annual Eligibility Report,
(Under the Provisions of Section 156,
Public Law 97–377), VA Form 21–8941.
OMB Control Number: 2900–0405.
Type of Review: Revision of a
currently approved collection.
Abstract: VA Form 21–8941 is
completed annually by claimants who
have earned income that is at or near the
limit of earned income. The Restored
Entitlement Program for Survivors
(REPS) program pays benefits to certain
surviving spouses and children of
Veterans who died in service prior to
August 13, 1981 or who died as a result
of a service-connected disability
incurred or aggravated prior to August
13, 1981. VA uses the information
collected to determine a claimant’s
continued entitlement to REPS benefits.
An agency may not conduct or
sponsor, and a person is not required to
respond to a collection of information
unless it displays a currently valid OMB
control number. The Federal Register
Notice with a 60-day comment period
soliciting comments on this collection

Abstract: Claimants complete VA
Form 21P–8416b to report
compensation awarded by another
entity or government agency for
personal injury or death. Such award is
consider as countable income; however,
medical, legal or other expenses
incident to the injury or death, or
incident to the collection or recovery of
the compensation may be deducted
from the amount awarded or settled.
The information collected is use to
determine the claimant’s eligibility for
income based benefits and the rate
payable.
An agency may not conduct or
sponsor, and a person is not required to
