The DA Form 3126–1 documents the agreement and becomes a contract signed by both the secondary institution and the U.S. Government. This form provides information on the school’s facilities and states specific conditions if a NDCC unit is placed at the institution. The data provided on the applications is used to determine which school will be selected. Affected Public: State, Local, or Tribal Government; Not-for-Profit Institution. Annual Burden Hours: 35.
