Television Systems’’ to solicit basic
operational information from the cable
television industry. The information
requested includes: the operator’s name
and address; system-wide capacity and
frequency information; channel usage;
and number of subscribers. The purpose
of the form is to require operational
cable television systems to verify,
correct and/or furnish the Commission
with the most current information on
their respective cable systems.

As part of its continuing effort
to reduce paperwork burdens, and as
required by the Paperwork Reduction
Act (PRA) of 1995 (44 U.S.C. 3501–
3520), the Federal Communications
Commission (FCC or the Commission)
invites the general public and other
Federal agencies to take this
opportunity to comment on the
following information collection.
Comments are requested concerning:
whether the proposed collection of
information is necessary for the proper
performance of the functions of the
Commission, including whether the
information shall have practical utility;
the accuracy of the Commission’s
burden estimate; ways to enhance the
quality, utility, and clarity of the
information collected; ways to minimize

is a consolidated application form for
Ship, Aircraft, Amateur, Restricted and
Commercial Radio Operators, and
General Mobile Radio Services and is
used to collect licensing data for the
Universal Licensing System. The
Commission is requesting OMB
approval for an extension (no change in
the reporting, recordkeeping and/or
third party disclosure requirements).
The Commission is making minor
clarifications to the instructions on the
main form and schedules B and E for
clarification purposes.
The data collected on this form
includes the Date of Birth for
Commercial Operator licensees however
this information will be redacted from
public view.
The FCC uses the information in FCC
Form 605 to determine whether the
applicant is legally, technically, and
financially qualified to obtain a license.

monthly as of the last calendar day of
the month from a stratified sample of
finance companies. Each monthly report
collects balance sheet data on major
categories of consumer and business
credit receivables and on major shortterm liabilities. For quarter-end months
(March, June, September, and
December), additional asset and liability
items are collected to provide a full
balance sheet. A supplemental section
collects data on securitized assets. The
data are used to construct universe
estimates of finance company holdings,
