Disability Benefits Questionnaire, will
be used for disability compensation or
pension claims which require an
examination and/or receiving private
medical evidence that may potentially
be sufficient for rating purposes. The
form will be used to gather necessary
information from a claimant’s treating
physician regarding the results of
medical examinations and information
related to the claimant’s diagnosis of a
shoulder or arm condition. VA will
gather medical information related to
the claimant that is necessary to
adjudicate the claim for VA disability
benefits.

opportunity for public comment on the
proposed collection of certain
information by the agency. Under the
Paperwork Reduction Act (PRA) of
1995, Federal agencies are required to
publish notice in the Federal Register
concerning each proposed collection of
information, including each new
collection, and allow 60 days for public
comment in response to the notice. This
notice solicits comments on information
needed to adjudicate the claim for VA
disability benefits related to a claimant’s
diagnosis of a back condition.

The Department of Veterans Affairs
(VA) gives notice under the Federal
Advisory Committee Act, 5 U.S.C. App.,
that the Research Advisory Committee
on Gulf War Veterans’ Illnesses will
meet on June 17 and 18, 2013, in room
230 at VA Central Office, 810 Vermont
Avenue, Washington, DC. The meeting
will start at 8 a.m. each day and adjourn
at 5:30 p.m. on June 17 and at 12:30
p.m. on June 18. All sessions will be
open to the public.
The purpose of the Committee is to
provide advice and make
recommendations to the Secretary of
Veterans Affairs on proposed research
studies, research plans and research
strategies relating to the health
consequences of military service in the
Southwest Asia theater of operations
