SUPPLEMENTARY INFORMATION: The notice
of a major disaster declaration for the
State of Ohio is hereby amended to

on an information collection

Immediate Delivery Application (Forms
3461 and 3461 ALT) and Simplified
Entry. This request for comment is being
made pursuant to the Paperwork
Reduction Act of 1995 (Pub. L. 104–13).

SUPPLEMENTARY INFORMATION: CBP
invites the general public and other
Federal agencies to comment on
proposed and/or continuing information
collections pursuant to the Paperwork
Reduction Act of 1995 (Pub. L. 104–13).
The comments should address: (a)
Whether the collection of information is
necessary for the proper performance of

the functions of the agency, including
whether the information shall have
practical utility; (b) the accuracy of the
agency’s estimates of the burden of the
collection of information; (c) ways to
enhance the quality, utility, and clarity
of the information to be collected; (d)
ways to minimize the burden including
the use of automated collection
techniques or the use of other forms of
information technology; and (e) the
annual cost burden to respondents or
record keepers from the collection of
information (total capital/startup costs
and operations and maintenance costs).
The comments that are submitted will
be summarized and included in the CBP
request for Office of Management and
Budget (OMB) approval. All comments
will become a matter of public record.
In this document CBP is soliciting
comments concerning the following
information collection:
Title: Entry and Immediate Delivery
Application and Simplified Entry.
OMB Number: 1651–0024.
Form Numbers: CBP Form 3461 and
Form 3461 ALT.
Abstract: All items imported into the
United States are subject to examination
before entering the commerce of the
United States. There are two procedures
available to effect the release of
imported merchandise, including
‘‘entry’’ pursuant to 19 U.S.C. 1484, and
‘‘immediate delivery’’ pursuant to 19
U.S.C. 1448(b). Under both procedures,
CBP Forms 3461 and 3461 ALT are the
source documents in the packages
presented to Customs and Border
Protection (CBP). The information
collected on CBP Forms 3461 and 3461
ALT allow CBP officers to verify that the
information regarding the consignee and
shipment is correct and that a bond is
on file with CBP. CBP also uses these
forms to close out the manifest and to
establish the obligation to pay estimated
duties in the time period prescribed by
law or regulation. CBP Form 3461 is
also a delivery authorization document
and is given to the importing carrier to
authorize the release of the
merchandise.
CBP Forms 3461 and 3461 ALT are
provided for by 19 CFR 141 and 142.

Simplified Entry is a program for ACE
entry summary filers in which importers
or brokers may file Simplified Entry
data in lieu of filing the CBP Form 3461.
This data consists of 12 required
elements: importer of record; buyer
name and address; buyer employer
identification number (consignee
number), seller name and address;
manufacturer/supplier name and
