Type of Review: Extension of a
currently approved collection.
Abstract: VA Form 21–8941 is
completed annually by claimants who
have earned income that is at or near the
limit of earned income. The REPS
program pays benefits to certain
surviving spouses and children of
veterans who died in service prior to
August 13, 1981 or who died as a result
of a service-connected disability
incurred or aggravated prior to August
13, 1981. VA uses the information
collected to determine a claimant’s
continued entitlement to Restored
Entitlement Program for Survivors
(RESPS) benefits.

Abstract: Claimants who wish to
waive VA disability benefits in order to
receive active or inactive duty training
pay are required to complete VA Forms
21–8951 and 21–8951–2. Active and
inactive duty training pay cannot be
paid concurrently with VA disability
compensation or pension benefits.
Claimants who elect to keep training
pay must waive VA benefits for the
number of days equal to the number of
days in which they received training
pay.
An agency may not conduct or

2545.

Type of Review: Extension of a
currently approved collection.
Abstract: VA Form 21–2545 is
completed by claimants prior to
undergoing a VA examination for
disability benefits. The examining
physician also completes the form to
record the findings of such examination.
An examination is required where the
reasonable probability of a valid claim
is indicated in any claims for disability
compensation or pension, including
claims for benefits based on the need of
a veteran, surviving spouse, or parent
for regular aid and attendance, and for
benefits based on a child’s’ incapacity of
self-support. VA uses the data to
determine the level of disability.
An agency may not conduct or
sponsor, and a person is not required to
respond to a collection of information
