OMB Control Number: 2900–0009. Type of Review: Extension of a currently approved collection. Abstract: VA Form 28–1900 is completed by Veterans with a combined service–connected disability rating of ten percent or more and awaiting discharge for such disability to apply for vocational rehabilitation benefits. VA provides service and assistance to veterans with disabilities, who have an entitlement determination, to gain and keep suitable employment. Vocational rehabilitation also provides service to support veterans with disabilities to achieve maximum independence in their daily living activities if employment is not reasonably feasible. VA uses the information collected to determine the claimant’s eligibility for vocational rehabilitation benefits. An agency may not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays a currently valid OMB control number.
