Abstract: VA Form 21–2545 is completed by claimants prior to undergoing a VA examination for disability benefits. The examining physician also completes the form to record the findings of such examination. An examination is required where the reasonable probability of a valid claim is indicated in any claims for disability compensation or pension, including claims for benefits based on the need of a veteran, surviving spouse, or parent for regular aid and attendance, and for benefits based on a child’s’ incapacity of self-support. VA uses the data to determine the level of disability. Affected Public: Individuals or households. Estimated Annual Burden: 45,000 hours. Estimated Average Burden per Respondent: 15 minutes.
