The Voucher and Payment Request Form is used by AmeriCorps members to request a payment from their Segal Education Award account; by schools and lenders to verify eligibility for the payment; and by both parties to satisfy certain legal requirements. Copies of the information collection request can be obtained by contacting the office listed in the ADDRESSES section of this notice.
