SUMMARY: The Department of the
Treasury, as part of its continuing effort
to reduce paperwork and respondent
burden, invites the general public and
other Federal agencies to take this
opportunity to comment on proposed
and/or continuing information
collections, as required by the
Paperwork Reduction Act of 1995,
Public Law 104–13 (44 U.S.C.
3506(c)(2)(A)). Currently, the IRS is
soliciting comments concerning Form
211, Application for Reward for Original
Information.

Form Number: Form 211.
Abstract: Form 211 is the official
application form used by persons
requesting rewards for submitting
information concerning alleged
violations of the tax laws by other
persons. Such rewards are authorized by
Internal Revenue Code section 7623.
The data is used to determine and pay
rewards to those persons who
voluntarily submit information.
Current Actions: Form 211(SP), the
Spanish version of Form 211 is being
discontinued. There are no changes
being made to form 211 at this time.
