The companies listed in this notice
have applied to the Board for approval,
pursuant to the Bank Holding Company
Act of 1956 (12 U.S.C. 1841 et seq.)
(BHC Act), Regulation Y (12 CFR Part
225), and all other applicable statutes
and regulations to become a bank
holding company and/or to acquire the
assets or the ownership of, control of, or
the power to vote shares of a bank or
bank holding company and all of the
banks and nonbanking companies
owned by the bank holding company,
including the companies listed below.
The applications listed below, as well
as other related filings required by the
Board, are available for immediate
inspection at the Federal Reserve Bank
indicated. The applications also will be
available for inspection at the offices of
the Board of Governors. Interested
persons may express their views in
writing on the standards enumerated in
the BHC Act (12 U.S.C. 1842(c)). If the
proposal also involves the acquisition of
a nonbanking company, the review also
includes whether the acquisition of the
nonbanking company complies with the
standards in section 4 of the BHC Act
(12 U.S.C. 1843). Unless otherwise
noted, nonbanking activities will be
conducted throughout the United States.

The companies listed in this notice
have given notice under section 4 of the
Bank Holding Company Act (12 U.S.C.
1843) (BHC Act) and Regulation Y (12
CFR Part 225) to engage de novo, or to
acquire or control voting securities or
assets of a company, including the
companies listed below, that engages
either directly or through a subsidiary or
other company, in a nonbanking activity
that is listed in § 225.28 of Regulation Y
(12 CFR 225.28) or that the Board has
determined by Order to be closely
related to banking and permissible for
bank holding companies. Unless
otherwise noted, these activities will be

PACHA
was established by Executive Order
12963, dated June 14, 1995 as amended
by Executive Order 13009, dated June
14, 1996. The Council was established
to provide advice, information, and
recommendations to the Secretary
regarding programs and policies
intended to (a) Promote effective
prevention of HIV disease, (b) advance
research on HIV and AIDS, and (c)
promote quality services to persons
living with HIV disease and AIDS.
PACHA was established to serve solely
as an advisory body to the Secretary of
Health and Human Services.
The meeting will be open to the
public through a conference call phone
number provided above. There will be
a limited amount of open lines for the
public; early registration is highly
recommended. Individuals who
participate using this service and who
need special assistance or other
reasonable accommodations, should
submit a request at least five days prior
to the meeting. Members of the public
who participate using the conference
call phone number will be able to listen
to the meeting but will not be heard
until the public comment period.

Centers for Medicare & Medicaid
Services (CMS) is publishing the
following summary of proposed
collections for public comment.

comments regarding this burden
estimate or any other aspect of this

plans). Cost Plans that are regulated
under Section 1876 of the Social
Security Act, and Employer Group
Waiver Plans (EGWP) may also provide
a Part D benefit. Organizations wishing
to provide services under the
Prescription Drug Benefit Program must
complete an application, negotiate rates,
and receive final approval from CMS.
Existing Part D Sponsors may also
expand their contracted service area by
completing the Service Area Expansion
(SAE) application. The collected
information will be used by CMS to: (1)
Ensure that applicants meet CMS
requirements, (2) support the
determination of contract awards. Form

Request: Revision of a currently

Information Collection: Part C Medicare
Advantage Application and 1876 Cost
Plan Expansion Application—CY 2012;
Use: The Balanced Budget Act of 1997
(BBA) established a new ‘‘Part C’’ in the
Medicare statute (sections 1851 through
1859 of the Social Security Act (the
Act)) which provided for a
Medicare+Choice (M+C) program.
Under section 1851(a)(1) of the Act,
every individual entitled to Medicare
Part A and enrolled under Part B, except
for most individuals with end-stage
renal disease (ESRD), could elect to
receive benefits either through the
Original Medicare Program or an M+C
plan, if one was offered where he or she
lived. The Medicare Prescription Drug,
Improvement, and Modernization Act of
2003 (MMA) established the Medicare
Prescription Drug Benefit Program (Part
D) and made revisions to the provisions
of Medicare Part C, governing what is
now called the Medicare Advantage
(MA) program (formerly
Medicare+Choice). Organizations
wishing to provide healthcare services
under MA and/or MA–PD plans must
complete an application, file a bid, and
receive final approval from CMS.
Existing MA plans may expand their
contracted area by completing the
Service Area Expansion (SAE)
