SUMMARY: The Department of the
Treasury, as part of its continuing effort
to reduce paperwork and respondent
burden, invites the general public and
other Federal agencies to take this
opportunity to comment on proposed
and/or continuing information
collections, as required by the
Paperwork Reduction Act of 1995,
Public Law 104–13 (44 U.S.C.
3506(c)(2)(A)). Currently, the IRS is
soliciting comments concerning Form
1040EZ–T, Claim for Refund of Federal
Telephone Excise Tax.

Telephone Excise Tax.

Form Number: 1040EZ–T.
Abstract: Form 1040EZ–T was
developed as a result of Notice 2006–50.
The purpose of the form is to allow
individuals that are not required to file
an individual income tax return to claim
a refund of the federal telephone excise
taxes paid. The taxes must have been
paid after February 28, 2003 and before
August 1, 2006. This form can only be
filed once.
Current Actions: There are no changes
being made to the form at this time.
