on the current or most recent
contractors, the Committee has
determined that the products listed
below are suitable for procurement by
the Federal Government under 41 U.S.C.
46–48c and 41 CFR 51–2.4.

not have a significant impact on a
substantial number of small entities.
The major factors considered for this

When members of the uniformed

incompetent, the need arises to have a
trustee appointed to act on their behalf
with regard to military pay matters.
Individuals will complete this form to
apply for appointment as a trustee on
behalf of the member. The requirement
to complete this form helps alleviate the
opportunity for fraud, waste and abuse
of Government funds and member’s
benefits.
