For the Reserve Components, the Military Service Personnel Office forwards the DD Form 2475 to the lending institution. For active-duty Service, the Service mails the form to the lending institution. This form is an essential element of a chaplain’s professional qualifications and will become a part of a chaplain’s military personnel record.
