Proposed Project
‘‘Coordinating Care Across Primary Care
and Specialty Care Practices’’
AHRQ proposes an evaluation of the
redesign of the transitions of care
between primary care and specialty care
services. The purpose of the redesign is
to remedy inefficiencies in the current
referral processes that threaten care
quality and safety, and system
efficiency. This redesign is being
