a total of approximately 68 of the 5,330
total responses per year being submitted
by mail. The average first-class postage
cost for a mailed submission will be 75

approximately $51 per year.
The total non-hour respondent cost
burden for this collection in the form of
filing fees and postage costs is estimated
to be $529,701 per year.
