Abstract: Claimants admitted to a VA
medical facility complete VA Form 10–
0137 to appoint a health care agent to
make decisions about his or her medical
treatment and to record specific
instructions about their treatment
preferences in the event they no longer
can express their preferred treatment.

meeting is open to the public.
The purpose of the Group is to advise
the Secretary of Veterans Affairs and the
Under Secretary for Health on the care
and treatment of disabled veterans, and
other matters pertinent to the
Department’s Veterans Health
Administration (VHA).
The agenda for the meeting will
include discussions of the evolving
relationship between VA and the
Department of Defense, an update on
mental health services, academic
affiliations, safety/quality initiatives, the
political climate, and VHA’s public
relations campaign.
Any member of the public wishing to
attend should contact Juanita Leslie,
