Overview of This Information
Collection
(1) Type of Information Collection:
Extension of a currently approved
collection.
(2) Title of the Form/Collection: SemiAnnual Progress Report for Grantees
from the Education and Technical
Assistance Grants to End Violence
Against Women with Disabilities
Program (Disability Grant Program).
(3) Agency form number, if any, and
the applicable component of the
Department of Justice sponsoring the
collection: Form Number: 1122–0012.
U.S. Department of Justice, Office on
Violence Against Women.
(4) Affected public who will be asked
or required to respond, as well as a brief
abstract: The affected public includes
the approximately 18 grantees of the

Disability Grant Program. Grantees
include states, units of local
government, Indian tribal governments
and non-governmental private entities.
These grants provide funds for
education and technical assistance in
the form of training, consultations, and
information to organizations and
programs that provide services to
individuals with disabilities and to
domestic violence programs providing
shelter or related assistance.
(5) An estimate of the total number of
respondents and the amount of time
estimated for an average respondent to
respond/reply: It is estimated that it will
take the approximately 18 respondents
(Disability Program grantees)
approximately one hour to complete a
semi-annual progress report. The semiannual progress report is divided into
sections that pertain to the different
types of activities in which grantees
may engage. A Disability Program
grantee will only be required to
complete the sections of the form that
pertain to its own specific activities.
(6) An estimate of the total public
burden (in hours) associated with the
collection: The total annual hour burden
to complete the data collection forms is
36 hours, that is 18 grantees completing
a form twice a year with an estimated
completion time for the form being one
hour.

of Firearms Ammunition and
Implements of War.

Department of Justice sponsoring the
collection: Form Number: ATF F 6, Part
1 (5330.3A) Bureau of Alcohol, Tobacco,
Firearms and Explosives.
(4) Affected public who will be asked
or required to respond, as well as a brief
abstract: Primary: Individuals or
households. Other: Business or other
for-profit, Federal Government, State,
Local or Tribal Government. The form is
used to determine whether firearms,
ammunition and implements of war are
eligible for importation into the United
States. It is also used to secure
authorization to import such articles
and serves as authorization to the U.S.
Customs Service to allow these articles
entry into the United States.
(5) An estimate of the total number of
respondents and the amount of time
estimated for an average respondent to
respond: It is estimated that 11,000
respondents will complete a 30 minute
form.
