0730a to request participation in the VA
child care subsidy program. VA will use

percentage of monthly costs to be
subsidized for child care.

child care provider. The data will be
used to determine whether the child
care provider is licensed and/or
regulated by the state to perform child
care.

AGENCY:
SUMMARY: The Veterans Health
Administration (VHA) is announcing an
opportunity for public comment on the
proposed collection of certain
information used by the agency. Under
the Paperwork Reduction Act (PRA) of
1995, Federal agencies are required to
publish notice in the Federal Register
concerning each proposed collection of
information, including each proposed
extension of a currently approved
collection, and allow 60 days for public
comment in response to the notice. This
notice solicits comments on information
needed to record patient’s specific
instructions about health care decisions
in the event he or she is no longer has
decision-making capability.
DATES: Written comments and

received on or before July 14, 2008.

data to carry out the claimant’s wish.
