requires applicants to file three
appropriately marked copies of an
application with the appropriate
Regional Office. The applications are to
clearly state the type of filing and
contain all exhibits and other pertinent
documents. Applications, notices, or
other filings that raise an issue of policy
or law require that two additional
copies be submitted to the Applications
Filing Room at OTS in Washington, DC.

Description: The OTS analyzes the
submitted information to determine
whether the applicant meets the
statutory and regulatory criteria to form
a mutual holding company and/or
perform minority stock issuances.
Information provided in the notice or
application is essential if the OTS is to
fulfill its mandate to prevent insider
abuse and unsafe and unsound practices
by mutual holding companies and their
subsidiaries. Minority issuances are not
feasible without an application process
