Individuals who are seeking information pertaining to them are directed to clearly mark the envelope and letter ‘‘Privacy Act Request.’’ Within the text of the request, the subject of the record must provide his/her account number and/or the full name, date and place of birth, and notarized signature, and any other information which may assist in identifying and locating the record, and a return address.
