If an applicant is unable to provide a birth certificate, the recruiter will forward a DD Form 372, ‘‘Request for Verification of Birth,’’ to a state or local agency requesting verification of the applicant’s birth date. This verification of birth ensures that the applicant does not fall outside the age limitations, and that the applicant’s place of birth supports the citizenship status claimed by the applicant. Needs and Uses: This information collection requirement is necessary to implement Public Law 95–202, section 401 (codified at 38 U.S.C. 106 Note) which directs the Secretary of Defense to determine if civilian employment or contractual service rendered by groups to the Armed Forces of the United States shall be considered active duty. This information is collected on DD Form 2168, ‘‘Application for Discharge of Member or Survivor of Member of PO 00000 Frm 00023 Fmt 4703 Sfmt 4703 6131 Group Certified to Have Performed Active Duty with the Armed Force of the United States’’ which provides the necessary information to assist each of the Military Departments in determining if an applicant was a member of a group which has performed active military service. Those individuals who have been recognized as a member of an approved group are eligible for benefits provided for by laws administered by the Veteran’s Administration.
