a. VA Form 10–5345 is used to obtain
a written consent from patients before
information concerning his or her
treatment for alcoholism or alcohol
abuse, drug abuse, sickle cell anemia, or
infection with the human
immunodeficiency virus (HIV) can be
disclosed to private insurance
companies, physicians, and other third
parties.
b. Patients complete VA Form 10–
5345 to request a copy of their medical
records from the Department of Veterans
Affairs.
An agency may not conduct or
sponsor, and a person is not required to
respond to a collection of information
unless it displays a currently valid OMB

the State Approving Agency.
OMB Control Number: 2900–0568.
Type of Review: Extension of a
previously approved collection.
Abstract: Accredited and
nonaccredited educational institutions,
with the exceptions of elementary and
secondary schools, must submit copies
of their catalog to State approving
agency when applying for approval of a
new course. State approval agencies use
the catalog to determine what courses
can be approved for VA training. VA
pays educational assistance to veterans,
persons on active duty or reservists, and
eligible persons pursuing an approved
program of education. Educational
assistance is not payable when
claimants pursue unapproved courses.
An agency may not conduct or
sponsor, and a person is not required to
respond to a collection of information
