Estimation of the total number of hours needed to prepare the information collection including number of respondents, frequency of response, and hours of response: The estimated total number of burden hours needed to prepare the information collection is 1,105,000; the number of respondents is 26,000 generating approximately 78,000 annual responses; the frequency of response is annually and every 5 years; and the estimated number of time needed to prepare the response varies from 1.25 hours to 40 hours. Status of the proposed information collection: Extension of a currently approved collection.
