Title: Payment and Reimbursement
for Emergency Services for Non ServiceConnected Conditions in Non-VA
Facilities.
OMB Control Number: 2900–0620.
Type of Review: Extension of a
currently approved collection.
Abstract: Veterans enrolled in VA’s
health-care system are personally liable
for emergency treatment rendered at
non-VA health facilities. Veterans or
their representative, and the health care
provider of the emergency treatment to
the veteran must submit a claim in
writing or complete a Health Insurance
Claim Form HCFA 1450 and 1500 or
Medical Uniform Institutional Provider
Bill Form UB–82 and 92 to request
payment or reimbursement for such
treatment. VA uses the data collected to
determine the claimant’s eligibility for
payment or reimbursement.
An agency may not conduct or
sponsor, and a person is not required to
respond to a collection of information
unless it displays a currently valid OMB
control number. The Federal Register
Notice with a 60-day comment period
soliciting comments on this collection
of information was published on June
22, 2005, at page 36234.

program to ensure veterans and
beneficiaries have information about
benefits and services to which they may
be entitled. To support the program, VA

distributes copies of publications to
Veterans Service Organizations’
representatives to be used in rendering
services and representation of veterans,
their spouses and dependents. Service
organizations complete VA Form 3215
to request placement on a mailing list
for specific VA publications.
b. Veterans or beneficiaries complete
VA Form 3288 to provide VA with a
written consent to release his or her
records or information to third parties
such as insurance companies,
physicians and other individuals.
c. VA Form Letter 70–2 is used to
obtain additional information from a
correspondent when the incoming
correspondence does not provide
sufficient information to identify a
veteran. VA personnel use the
information to identify the veteran,
determine the location of a specific file,
and to accomplish the action requested
by the correspondent such as processing
a benefit claim or file material in the
individual’s claims folder.
d. Title 38 U.S.C.5701(f)(1) authorized
the disclosure of names or addresses, or
both of present or former members of
the Armed Forces and/or their
beneficiaries to nonprofit organizations
(including members of Congress) to
notify veterans of Title 38 benefits and
to provide assistance to veterans in
obtaining these benefits. This release
includes VA’s Outreach Program for the
purpose of advising veterans of non-VA
Federal, State and local benefits and
programs.

studies, the adequacy of the protocols,
and the scientific validity and propriety
of technical details, including
protection of human subjects.
The sessions each day will open to
the public from 8 a.m. to 8:30 a.m. for
the discussion of administrative matters
and the general status of the program.
The sessions will be closed from 8:30
a.m. to 3 p.m. for the Board’s review of
research and development applications.
During the closed sessions of the
meeting, discussions and
recommendations will deal with
qualifications of personnel conducting
the studies, staff and consultant
critiques of research proposals and
similar documents, and the medical
records of patients who are study
subjects, he disclosure of which would
constitute a clearly unwarrantwed
invasion of personal privacy. As
provided by section 10(d) of Public Law
92–463, as amended, closing portions of
this meeting is in accordance with 5
U.S.C. 552b(c)(6) and (c)(9)(B).
