(5) An estimate of the total number of respondents and the amount of time estimated for an average respondent to respond: It is estimated that 26 persons complete the Prescription Monitoring Program Questionnaire on paper and 25 electronically, at 5 hours per form, for an annual burden of 255 hours. (6) An estimate of the total public burden (in hours) associated with the collection: It is estimated that there are 255 burden hours associated with this collection. The proposed information collection is published to obtain comments from the public and affected agencies.
