Surviving Spouses, VA Form 26–1817.
OMB Control Number: 2900–0055.
Type of Review: Extension of a
currently approved collection.
Abstract: VA Form 26–1817 is
completed by unmarried surviving
spouse of veterans as a formal request
for a certificate of eligibility for home
loan benefits. An unmarried surviving
spouse may be entitled to home loan
benefits if the veteran’s death occurred
while serving on active duty or was a
direct result of service-connected
disabilities. VA uses the data collected
to verify the veteran’s service-connected
death and status of the applicant as
unmarried surviving spouse.
An agency may not conduct or
sponsor, and a person is not required to
respond to a collection of information
unless it displays a currently valid OMB
control number. The Federal Register
Notice with a 60-day comment period
soliciting comments on this collection
of information was published on
January 25, 2005, at pages 3581–3582.

comments on: (1) Whether the proposed
collection of information is necessary
for the proper performance of VBA’s
functions, including whether the
information will have practical utility;
(2) the accuracy of VBA’s estimate of the
burden of the proposed collection of
information; (3) ways to enhance the
quality, utility, and clarity of the
information to be collected; and (4)
ways to minimize the burden of the
collection of information on
respondents, including through the use
of automated collection techniques or
the use of other forms of information
technology.

other Conveyance and Adaptive
Equipment (under 38 U.S.C. 3901–

Abstract: Veterans and servicepersons
complete VA Form 21–4502 to apply for
automobile or other conveyance
allowance, and reimbursement for the
cost and installation of adaptive
equipment. The claimants must possess
one of the following disabilities that

resulted from injury or a disease that
was incurred or aggravated during
active military service: (1) Loss or
permanent loss of use of one or both
feet, or hands; (2) permanent
impairment of vision in both eyes with
a central visual acuity of 20/200 or less
in the better eye with corrective glasses,
or central visual acuity of more than 20/
200 if there is a field defect in which the
peripheral field had contracted to such
an extent that the widest diameter of
visual field has an angular distance no
greater than 20 degrees in the better eye.
VA uses the information to determine
the claimant’s eligibility for such
benefits.
