SUMMARY: Under the provisions of the
Paperwork Reduction Act of 1995 (44
U.S.C. Chapter 35), the General Services
Administration will be submitting to the
Office of Management and Budget
(OMB) a request to review and approve
a renewal of a currently approved
information collection requirement
regarding transfer order-surplus
personal property and continuation
sheet.
Public comments are particularly
invited on: Whether this collection of
information is necessary and whether it
will have practical utility; whether our
estimate of the public burden of this
collection of information is accurate,
and based on valid assumptions and
methodology; ways to enhance the
quality, utility, and clarity of the
information to be collected.

A. Purpose
Standard form (SF) 123, Transfer
Order-Surplus Personal Property and
Continuation Sheet is used by public
agencies, nonprofit educational or
public health activities, programs for the
elderly, service educational activities,
and public airports to apply for
donation of Federal surplus personal
property. The SF 123 serves as the
transfer instrument and includes item
descriptions, transportation

(OWH) is seeking a new clearance to

associated with the evaluation of funded
programs. The evaluation is designed to
determine best practices and clearly
define the gender-centered approach to
HIV/AIDS prevention. The HIV/AIDS
programs to be evaluated are the Model
Mentorship. Incarcerated/Newly
released women and HIV prevention in
the rural south. The program consists of
individual community-based
organizations from across the country.
The evaluation results will assess the
effectiveness of OWH reaching its
overarching HIV program goals of
increasing HIV prevention knowledge
and reducing the risk of contacting HIV
among young minority women.
