SUMMARY: The Department of the Treasury, as part of its continuing 
effort to reduce paperwork and respondent burden, invites the general 
public and other Federal agencies to take this opportunity to comment 
on proposed and/or continuing information collections, as required by 
the Paperwork Reduction Act of 1995, Public Law 104-13 (44 U.S.C. 
3506(c)(2)(A)). Currently, the IRS is soliciting comments concerning 
Form 8889, Health Savings Accounts (HSAs).

    Abstract: Form 8889 is used to report contributions to and 
distributions from an HSA. The form will figure any HSA deduction that 
is entered on Form 1040. The form will also figure the amount of any 
taxable distributions from an HSA that is entered on Form 1040, and any 
distributions that are subject to the additional 10% tax that is 
entered on Form 1040.
    Current Actions: There are no changes being made to the forms at 
this time.
    Type of Review: Extension of a currently approved collection.
    Affected Public: Individuals or households.
    Estimated Number of Respondents: 1,400,000.
    Estimated Time Per Respondent: 29 min.
