VI.3. Reporting Requirements
You must provide CDC with an
original, plus two hard copies of the
following reports:
1. Interim progress report, due no less
than 90 days before the end of the
budget period. The progress report will
serve as your non-competing
continuation application, and must
contain the following elements:
a. Current Budget Period Activities
Objectives.
b. Current Budget Period Financial
Progress.
c. New Budget Period Program
Proposed Activity Objectives.
d. Budget.
e. Measures of Effectiveness.
f. Additional Requested Information.
2. Financial status report and annual
progress report, no more than 90 days
after the end of the budget period.
3. Final financial and performance
reports, no more than 90 days after the
end of the project period.
These reports must be mailed to the
Grants Management Specialist listed in
the ‘‘Agency Contacts’’ section of this
announcement.

In accordance with section 10(a)(2) of
the Federal Advisory Committee Act
(Pub. L. 92–463), the Centers for Disease
Control and Prevention (CDC), National
Center for Health Statistics announces
the following committee meeting.
Name: Board of Scientific Counselors,
NCHS.
Times and Dates: 2 p.m.–5:30 p.m., April
21, 2005. 8:30 a.m.—2 p.m., April 22, 2005.
Place: NCHS Headquarters, 3311 Toledo
Road, Hyattsville, Maryland 20782.
Status: Open to the public, limited only by
the space available. The meeting room
accommodates approximately 100 people.
Purpose: This committee is charged with
providing advice and making
recommendations to the Secretary; the
Director, CDC; and Director, NCHS, regarding
the scientific and technical program goals
and objectives, strategies, and priorities of
NCHS.
Matter To Be Discussed: The agenda will
include welcome remarks by the Director,
NCHS; introductions of members and key
NCHS staff; scientific presentations and
discussions; and an open session for
comments from the public. Requests to make
an oral presentation should be submitted in
writing to the contact person listed below by
close of business, March 25, 2005. All
requests to make oral comments should
contain the name, address, telephone
number, and organizational affiliation of the
presenter. Written comments should not
exceed five single-spaced typed pages in
length and should be received by the contact

In compliance with the requirement
of section 3506(c)(2)(A) of the
Paperwork Reduction Act of 1995, the
Centers for Medicare & Medicaid
Services (CMS), Department of Health
and Human Services, is publishing the
following summary of proposed
collections for public comment.
Interested persons are invited to send
comments regarding the burden
estimates or any other aspects of these
collections of information, including
any of the following subjects: (1) The
necessity and utility of the proposed
information collection for the proper
performance of the agency’s functions;
(2) the accuracy of the estimated
burden; (3) ways to enhance the quality,
utility, and clarity of the information to
be collected; and (4) the use of
automated collection techniques or
other forms of information technology to
minimize the information collection
burden.
We are, however, requesting an
emergency review of the information
collections referenced below. In
compliance with the requirement of
section 3506(c)(2)(A) of the Paperwork
Reduction Act of 1995, we have
submitted to the Office of Management
and Budget (OMB) the following
requirements for emergency review. We
are requesting emergency review
because these collections of information
are needed before the expiration of the
normal time limits under OMB’s
regulations at 5 CFR part 1320. This is
necessary to ensure compliance with an
initiative of the Administration. We
cannot reasonably comply with the
normal clearance procedures because
the normal procedures are likely to
cause a statutory deadline to be missed.
