Abstract of proposed collection: Form
DS–3057 is designed to collect medical
information to provide medical
providers with current and adequate
information to base decisions on
whether a federal employee and family
members will have sufficient medical
resources at a diplomatic mission
abroad to maintain the health and
fitness of the individual and family
members.
Methodology: The information
collected will be collected through the
use of an electronic forms engine or by
hand written submission using a preprinted form.

seq.), Delegation of Authority No. 234 of
October 1, 1999, Delegation of Authority
No. 236–3 of August 28, 2000 (and, as
appropriate, Delegation of Authority No.

determine that an object to be included
in the exhibition ‘‘The Martyrdom of
Saint Ursula,’’ imported from abroad for
temporary exhibition within the United
States, is of cultural significance. The
object is imported pursuant to a loan
agreement with the foreign owner or
custodian. I also determine that the
exhibition or display of the exhibit
object at The Metropolitan Museum of
Art, New York, New York, from on or
about April 10, 2017, until on or about
June 30, 2017, and at possible additional
exhibitions or venues yet to be
determined, is in the national interest.

signed April 17, 2013 by the Town of
Stratford, City of Bridgeport,
Connecticut Department of
Transportation, Connecticut Office of
Policy and Management, and the
Federal Aviation Administration.
There are two subject parcels
identified in the Intergovernmental
Agreement. The first property is the
Long Beach Parcel that is located to the
southwest of the end of Runway 6 and
is 33.98 acres. The second property
known as Parcel 16, is located to the
northwest of Runway 11 and is 7.65
acres. The two parcels will be
transferred to the Town of Stratford, CT
at a fair market value of $486,600. The
parcels will be transferred with
easements to protect the airport. Given
the location of the two parcels, the
disposal of this property will have no
effect on aviation land nor future
development opportunities for the
airport. The proceeds of the disposal,
per the Intergovernmental Agreement,
will be used to reduce the overall
Runway Safety Area project grant
amount and fund a portion of the City’s
match for the Runway Safety Area
project.
